Illinois developed the Supportive Living Program (SLP) as a cost-effective alternative to Nursing Home Care for low-income older persons and persons with physical disabilities who are currently under the Medicaid program.

By offering personal care and other services, residents can live independently and certainly take part in their care and decision making.

Personal choice, dignity, privacy, and individuality are highly emphasized along with person centered service planning.

The Department of Healthcare and Family Services (HFS) has obtained a “waiver” to allow payment for services that are not routinely or typically covered by Medicaid.

These include but are not limited to:

  • Personal Care
  • Homemaking
  • Laundry
  • Medication Assistance
  • Social and Health Activities
  • 24-hour response staff to meet residents scheduled and unscheduled needs

** The resident is responsible for paying the cost of room and board at the community, please see our Sales and Marketing Director for more information. **
