LEADERSHIP, MANAGERS AND TEAM MEMBERS:

Stonebridge’s commitment to independent living in a Supportive Living residential setting starts at the top. Our Executive Director and the management team are experienced professionals working together to encourage independent living and to connect residents with the external and on-site resources they need. The shared goal is for all Stonebridge residents to live beyond disabilities that need not be a barrier to their own unique journey at continuing a purposeful life.

Our Wellness leaders are comprised of licensed Registered Nurses (RNs) and LPNs, who along with our 24/7 staffing of CNAs are there to assist residents. Each Stonebridge resident has their own Individualized Healthcare Service Plan that’s developed as a partnership between the Wellness staff and the resident.

Our residents’ continued journeys at living a purposeful life is supported by our Director of Residence Life who will also still provide the fun and creative activities, outings, and events our residents enjoy. Our Stonebridge Social Services Director is proactive in assisting residents in accessing services from federal, state and local agencies and providers. Our Stonebridge Dietary Operations Team create and serve delicious and nutritious meals three times daily, served in a formal dining hall. The Maintenance and Housekeeping staff keep Stonebridge running smoothly and looking good. Our friendly and engaging Marketing Director and Move-In Coordinator work as a team to educate applicants and family members on the steps required to become a Stonebridge resident, and guide them through the process. Members of the management team work with the Stonebridge Business Office to assist residents with insurance items, and state and federal entitlement program issues. We all support the Move-In Coordinator in ensuring all residents stay in compliance with the federal tax credit program. Stonebridge has 12 Private Pay apartments and those applicants and residents receive the same support for their unique journey at continuing to live purposeful lives at Stonebridge.
