ILLINOIS SUPPORTIVE LIVING PROGRAM:

Illinois developed the Supportive Living Program (SLP) as a cost-effective alternative to Nursing Home Care for low-income adults with physical disabilities, who are currently under the Medicaid Program.

By offering personal care and other services, residents can live independently and take part in their care and decision making.

Personal choice, dignity, privacy, and individuality are highly emphasized along with person centered service planning.

These Services are included but are not limited to:

  • Personal Care
  • Housekeeping
  • Laundry
  • Medication Assistance
  • Social and Health Activities
  • 24-hour response staff to meet residents scheduled and unscheduled needs
